Complete or modify the form in accordance with the instructions below.

• Member Statement Name Order: This value determines how the Member's name will appear on Statements.
• Show All Purchases:  If enabled, this value expands the account statement to display purchases, regardless of the method of payment.
• Show F&B Details:  If enabled, all line items from F&B invoices will be displayed on the member's statement.
• Show Retail Details:  If enabled, all line items from retail invoices will be displayed on the member's statement.
• Show Member Billing Details:  If enabled, all line items from member billing invoices will be displayed on the member's statement.
• Show Web Details:  If enabled, all line items from web invoices invoices will be displayed on the member's statement.
• Group AR By Family Name: Deprecated
• Show Dependent Member Purchaser Name:  If enabled, the name of the dependent member will appear below the line item on the member statement.
• Statement Footer Messages:  These 2 fields allow the customization of the Member Account statement footer messages.
